Pediatric Therapy Bridge understands that the information families share with us — including developmental history, evaluation results, and treatment information — is sensitive. This page explains how we handle health-related information and the steps we take to protect it.
Our Approach to Health Information
Pediatric Therapy Bridge is a private-pay pediatric occupational therapy practice. We do not bill insurance directly on your behalf; instead, we provide a superbill that you may submit to your insurance carrier for possible out-of-network reimbursement.
Whether or not our practice meets the technical definition of a “covered entity” under the Health Insurance Portability and Accountability Act (HIPAA), we are committed to handling health and developmental information about your child with the same level of care, confidentiality, and security that HIPAA is designed to ensure.
Information We Collect Through Intake and Related Forms
Through our Intake Form, Client Contract, School Inquiry Form, and Consent to Communicate form, we may collect:
- Your child’s name, date of birth, and developmental or medical history
- Parent/guardian contact information
- Information relevant to scheduling an evaluation or treatment
- School or caregiver information, where applicable
These forms are currently hosted through a third-party form provider (JotForm).
How We Use and Protect This Information
We use the information collected only to:
- Schedule and provide occupational therapy evaluations and treatment
- Communicate with parents/guardians about their child’s care
- Prepare superbills for insurance reimbursement purposes
- Maintain records as required under Florida occupational therapy licensing regulations
We limit access to this information to our therapists and administrative staff who need it to provide your child’s care, and we do not sell or share this information for marketing purposes.
Confidentiality Under Florida Law
Independent of HIPAA, healthcare practitioner records in Florida are protected under Florida Statute § 456.057, which governs the confidentiality of patient records maintained by licensed healthcare practitioners. We handle records in accordance with this and other applicable Florida requirements.
Your Rights
You may:
- Request to review or receive a copy of your child’s records maintained by our practice
- Request corrections to information you believe is inaccurate
- Ask questions about how your child’s information is used or shared
- Request that we communicate with you through a specific method or address (as covered in our Consent to Communicate form)
To make any of these requests, please contact us using the information below.
Sharing of Information
We do not disclose information about your child except:
- To you, as the parent/guardian
- To school staff or caregivers, only with your consent (see our Consent to Communicate form)
- To our third-party service providers (such as our form and portal providers) solely to operate our practice
- When required by law, such as mandatory reporting obligations related to child safety
