a. Information You Provide Directly
We may collect information you voluntarily submit through the Site, including but not limited to:
- Contact Us form: name, email address, phone number, and message content
- Intake Form: child’s name, date of birth, and developmental/medical history; parent/guardian name and contact information; insurance/superbill information (we are private pay and do not bill insurance directly); and other information relevant to scheduling an evaluation
- Client Contract and Consent to Communicate forms: parent/guardian signature, contact preferences, and related consents
- School Inquiry Form: school name, contact information, and information about the child’s needs
- Client Portal sign-up and login: username, password, and account information
- Careers/Join Our Team inquiries: resume, contact details, and application information
b. Information Collected Automatically
When you browse the Site, we (and our service providers) may automatically collect certain information, including:
- IP address, browser type, device type, and operating system
- Pages visited, time spent on pages, and referring/exit URLs
- General geographic location (inferred from IP address)
- Cookies and similar tracking technologies (see Section 5 below)
c. Information from the Client Portal
If you sign up for our client portal, we or our portal provider may collect and store scheduling information, session notes, billing/superbill documentation, and secure messages exchanged with our therapists.

- Respond to inquiries and schedule evaluations or therapy sessions
- Provide occupational therapy services and communicate with parents/guardians about a child’s care
- Process intake, consent, and client contract documentation
- Operate and maintain the client portal
- Generate superbills for out-of-network insurance reimbursement purposes
- Improve the Site and our services
- Send administrative communications (appointment reminders, policy updates, billing notices)
- Comply with legal, licensing, and record-keeping obligations applicable to occupational therapy practices
- With your consent, send newsletters or promotional communications (you may opt out at any time)
We do not sell your personal information or your child’s information to third parties.
3. Protected Health Information (HIPAA)
Information collected in connection with your child’s evaluation and treatment (such as intake forms, session notes, and developmental history) may constitute Protected Health Information (“PHI”) under the Health Insurance Portability and Accountability Act (“HIPAA”). Our use and disclosure of PHI is governed by our Notice of Privacy Practices, which is provided separately at intake and available upon request. Where this Privacy Policy and our Notice of Privacy Practices differ with respect to PHI, the Notice of Privacy Practices governs.
4. How We Share Information
We may share information with:
- Service providers who perform functions on our behalf, such as website hosting, client portal/practice management software, email and communication platforms, and payment processing. These providers are only permitted to use your information to provide services to us.
- Business associates, where required under HIPAA, subject to a Business Associate Agreement.
- Legal or regulatory authorities, when required by law, court order, or to protect the safety of a child or others.
- Professional advisors (e.g., accountants, attorneys) as needed to operate our practice.
We do not share personal information with third parties for their own independent marketing purposes.
5. Cookies and Analytics
The Site may use cookies and similar technologies to improve functionality and understand how visitors use the Site. You can control cookies through your browser settings; disabling cookies may affect certain Site features.

Our services are directed to parents and legal guardians, not to children. Information about a child (such as intake or developmental information) is submitted to us by a parent or guardian, not collected directly from the child through the Site. We do not knowingly collect personal information directly from children under 13 through the Site in a manner inconsistent with the Children’s Online Privacy Protection Act (COPPA).
9. Your Choices and Rights
- Communications: You may opt out of non-essential email communications by using the unsubscribe link or contacting us directly.
- Access/Correction: You may request access to, or correction of, your or your child’s information by contacting us. Requests regarding clinical/medical records will be handled in accordance with our Notice of Privacy Practices and applicable law.
- Portal Account: You may update account information directly through the client portal or by contacting us.
10. Third-Party Links
The Site may link to third-party websites and resources (for example, Step Up For Students, ABCmouse, or The Masgutova Method). We are not responsible for the privacy practices of these third-party sites and encourage you to review their privacy policies separately.
11. Florida Residents
Pediatric Therapy Bridge operates in Miami-Dade and Broward Counties, Florida. We handle personal information in accordance with applicable Florida law, including the Florida Digital Bill of Rights, to the extent it applies to our practice.
